The first step is to complete an application and provide all required documentation to the staff. Once we receive all required documents we can get you on the way to becoming a member of the Hope family.

Once you have completed the application you can drop it by the Clinic at
411 E. Jefferson. If you have any question please do not hesitate to
call and ask for assistance. 972-923-2440.
When you bring your application to the Clinic you must also bring proof
of income (current Income Tax Return) and proof of residency (must be in
form of utility bill in your name a driver's licenses will not fulfill
this requirement). We will also need copies of any insurance cards you
have, including Medicaid, Medicare and CHIP.
The sooner you provide us with all the required information, the quicker
we can get you an appointment.
